| Carefully remove cotton swab from packet – avoid touching the cotton pad with your fingers. |

| Holding onto the wooden shaft, insert the pad end of the swab into your dog’s mouth. |

|
| Pinch the cheek around the swab and swirl around the pinched cheek for at least 10 seconds. |

| Carefully insert the swab (head first) back into the packet and seal – avoid touching the cotton pad with your fingers. Repeat Steps 1-4 with second swab. |

|
| Place the samples and application form in the reply paid envelope provided and post. |
