Photograph Upload Instructions

Please read the following before uploading your photograph

	Photographs can only be submitted when you have paid for and received your kit as you need the unique barcode as a reference.
	The Barcode Number can be found on the bottom right hand corner of the Application form, the back of the swabs and in the outside base of the bag the kit was in.
	Your photograph should be renamed to the same number as the barcode and your surname, ie B0000000004TURNER.
	Only JPEG files will be accepted
	Digital photo must not exceed 1 MB in size
	The photograph in the doggy passport will be an exact reproduction of the photograph provided.
	Photograph must be sized to 3.5 cm X 3.5 cm.

Please check all your details carefully before uploading the photograph, unless all the details match the details on your application form and the file has been renamed correctly, your photograph may not be printed on your doggy passport.
